Specialized ServicesWe provide, at no additional charge, three months of aftercare for those who complete their treatment plan in any of our levels of care. Each level of care can be accessed directly, with continuity of care referred back to the client’s home area and referring agency whenever possible. All treatment and discharge planning is done in consultation with insurance and managed care case managers. Individualized Case Management assures close coordination and contact with the referral source when appropriate, particularly with employers, EAP managers, other treatment providers, and legal counsel. Our programs are able to individualize a Treatment Plan for the special needs of each client. We have experience in a diverse special-needs population including, but not limited to, multiple relapse, secondary dual diagnosis, family dysfunction, ACOA, residual family-of-origin blocks to recovery, alternative lifestyles, law enforcement and public service personnel, poly and cross addictions, job jeopardy, safety-sensitive employment, Department of Transportation regulations, and legal problems. The Clinical Pathways format initiates discharge and treatment planning within the admission assessment process, utilizing admission criteria based on medical/clinical needs, psycho-social factors, client’s motivation for change, and financial resources. We believe that these cost effective sub-acute inpatient rehabilitation oriented levels of care provide the “bridge” between higher and lower levels of care, something that people seem to be looking for at a cost that is fair to insurance carriers, subscribers, and providers, and puts the patient’s care back into the equation.
